Medical Benefits

Our health care plans are designed to provide our benefit eligible team members with comprehensive coverage and choices. All plans are administered by United HealthCare (UHC). This year, we will offer three (3) medical plans:
  • Choice Plus HSA
  • Choice 3000
  • Surest Zero Deductible New
    • The Surest Zero Deductible Plan will replace the prior Choice EPO 1500 Plan, giving team members access to a more simplified healthcare experience with greater cost transparency and predictability.
Passive Enrollment Remains the Same: For most benefits, passive enrollment remains in place. If you’re happy with your current elections, most of your benefit choices will automatically roll over into the new plan year. The exception is team members currently enrolled in the Choice EPO 1500 Plan, who will automatically transition to the Surest Zero Deductible Plan if no action is taken during Open Enrollment.
Preventative Care Coverage: All three of our medical plans cover eligible preventive care services at no cost to you, even before you meet your deductible. This includes services like annual physicals, routine screenings, and immunizations when you use in-network providers.  Click here to learn more: What’s preventive care and what’s covered?
