Dental Benefits
Sonny’s Enterprise offers two dental plans designed to provide our benefit eligible employees with care to maintain their oral health. Generally, you can only change your benefit choices during the annual enrollment period. However, you may be able to change your coverage throughout the year if you experience a Qualified Family Status Change. Dental coverage is offered through United HealthCare Dental.

Dental HMO– Exclusive Network Plan Benefits
The Dental HMO provides a wide variety of benefits through participating providers. To search for a provider, go to MyUHC.com or call Member Services Department at 800-955-4137. At the time of service, you pay the dentist for any applicable copayments according to your schedule of benefits. No claims to file. No Maximums.
Dental PPO – UHC National Options – PPO 30 Network
